Scholastic Verification

Public or private institutions/organizations may request for the verification of the scholastic records of current and former students of the UP Open University.

Requirements:

  1. Verification form of the verifying institution.
  2. Duly signed authorization of the student for the release of his/her university records.
  3. Verification fee payment receipt amounting to Php 50.00. View the payment procedures here.
  4. Courier fee payment receipt amounting to PhP 190.00 if the verifying institution wishes to secure the original copy of the verified form. View the payment procedures here. If the scanned copy of the verified form is sufficient, courier fee is not needed.

Scholastic Verification Procedure:

Step 1: Mail or bring all requirements to:

or email scanned copies to [email protected]. Requests through phone calls are not accommodated.

Step 2: Wait for an email from the Office of the University Registrar for the result of the request.
