Application for Honorable Dismissal

Applying for an honorable dismissal would mean a voluntary withdrawal from the program and is applicable only to non-graduating students. However, if the student has a plan of enrolling again to any UP units, s/he should not request for a certificate of honorable dismissal. Students who are applying for honorable dismissal must write a letter to the University Registrar and file for university clearance.

How to Apply for Honorable Dismissal:

Step 1: Accomplished University Clearance Form. Indicate ‘Honorable Dismissal’ as the reason for clearing. Attach the following documents:

  • Signed letter addressed to the University Registrar citing the reason/s for withdrawing from the program.
  • A government-issued ID.

Step 2. The clearance shall be only approved by the Office of the University Registrar (final approver) if the following units have cleared the student of any deficiency/ies:

  • Office of the Student Affairs
  • University Library
  • Faculty Office
  • Records Section of the Office of the University Registrar

Step 3: Student receives email notification if the clearance submitted has been approved.
